What it is

The most-used oral steroid in the recreational world — mild reputation, strength and hardness with little water, popular in cutting runs and among women because virilization risk is lower than any other AAS (lower, not zero).

How it works

Oxandrolone is a 17-alpha-alkylated DHT derivative: the 17-alpha alkylation is what gives it oral activity and also what drives its hepatic risk. It belongs to the hepatotoxic 17-alpha-alkylated subgroup of oral anabolic steroids — a subgroup, not every orally active androgen. It does not aromatise, so there is no estrogen pathway, and its androgen-receptor binding is modest, which is the pharmacological basis of its "mild" reputation. That reputation is the problem: whatever it means, it does not extend to lipids or to suppression, and almost every misunderstanding of this drug comes from assuming it does.

What human evidence shows

One of the few AAS with real modern trial history (burn recovery, HIV wasting) — it preserves and adds lean mass. The "mild" label refers to androgenic side effects, not to safety: it is a 17-alpha-alkylated oral, which is exactly the liver-loading class.

The studies, one by one

  • This is, unexpectedly, one of the best-studied anabolic steroids in humans — because it had real clinical indications and real trials rather than a purely non-medical history.
  • Severe-burn medicine is the deepest literature: controlled studies in burn patients, including children, showed improved lean-mass retention, better wound healing and shorter hospital stays. That is high-quality human evidence in a hospital population — and the FDA nonetheless concluded in 2023 that the benefit-risk profile did not support keeping the drug approved, citing peliosis hepatis, sometimes-fatal liver tumours and cholestatic hepatitis.
  • It carried FDA indications for weight regain after significant loss, and was studied in HIV wasting, alcoholic hepatitis, Turner syndrome and Duchenne muscular dystrophy — a genuinely broad clinical record.
  • The burn literature also produced the clearest lipid warning: HDL suppression was marked and consistent, in supervised patients, at therapeutic exposures.
  • What does not exist is a trial of physique use in healthy adults. The body-composition effect transfers; the safety context of a monitored hospital patient does not.

Half-life and how long it lasts

An oral with a short half-life — roughly nine to ten hours, far shorter than injectable esters — so blood levels rise and fall within a day rather than persisting for weeks. That short window is why people wrongly assume its effects on lipids and the hormonal axis are equally short-lived; suppression and HDL depression build over the course of use and do not clear on the same schedule as the drug. Detection in anti-doping testing outlasts the pharmacological effect substantially.

Risks

Lipids get wrecked fast — HDL suppression from orals is among the worst per-milligram. Hepatic strain (17aa), full HPTA suppression like every AAS, hair loss in the predisposed, and for women virilization that is dose- and duration-dependent and partly irreversible. Unregulated products can be mislabelled or contaminated, and composition cannot be verified from the label.

Who should never touch it

  • Anyone with existing liver disease or an unexplained enzyme elevation — the FDA specifically cited peliosis hepatis and hepatic tumours when withdrawing approval.
  • Anyone with an already poor lipid profile, familial hyperlipidaemia, or cardiovascular disease.
  • Anyone taking warfarin or another anticoagulant without a prescriber managing the interaction.
  • Women who are pregnant, could become pregnant, or are unwilling to accept a non-zero risk of permanent voice change.
  • Anyone hoping to preserve fertility in the near term.

Interactions worth knowing

  • Anticoagulants — oxandrolone has one of the better-documented warfarin interactions in this class, capable of substantially increasing anticoagulant effect. This is a genuine clinical interaction, not a theoretical one.
  • Concurrent hepatotoxic exposures can increase risk; no safe combination has been established.
  • Diabetes medication — glucose control can shift.
  • Alcohol and other hepatically-cleared drugs add to the same load the alkylation already imposes.

What a clinician would watch

  • Lipid panel — the headline. HDL suppression is disproportionate to this drug's gentle reputation and shows up quickly and reliably.
  • Liver enzymes (ALT/AST) plus GGT and bilirubin — ALT and AST alone are ambiguous in people who lift, since training raises them without any liver problem; GGT helps separate the two.
  • Total and free testosterone, LH and FSH — suppression happens at ordinary exposures despite persistent claims that it does not.
  • Blood pressure.
  • In women, virilisation signs specifically: voice change is the one that does not reverse, and it is monitored by listening rather than by lab.

What stopping looks like

The compound itself clears quickly, but the two things that matter lag well behind it. Lipid abnormalities can persist after the drug is gone, with no established recovery timeline. The hormonal axis follows its own course, and because people are told this drug is mild, the low-testosterone window afterwards frequently arrives unexpected — flat mood, low libido, fatigue in someone who was assured none of this applied to them.

Myth vs evidence

It is mild, so it does not suppress you.

Suppression is documented at ordinary exposures. "Mild" refers to liver stress relative to other orals and to virilisation risk relative to other androgens — it was never a statement about the hormonal axis, and treating it as one is how people end up in an unplanned recovery.

It is the safe one for women.

It is the most-studied option in women, but no reliable ranking establishes it as the safest — and virilisation still occurs. Voice deepening and some other virilising changes can persist or be irreversible.

Its short half-life means the side effects clear quickly too.

The drug clears in hours; the lipid and axis effects it caused do not, and no reliable recovery timeline has been established. Different clocks entirely.

It is barely a steroid — more like a strong supplement.

It is a Schedule III anabolic steroid with genuine clinical potency, an FDA indication history, and the harshest lipid profile relative to its reputation of anything in this catalog.

Legal status

US: Schedule III anabolic steroid under the CSA — and as of 2023 there is NO FDA-approved oxandrolone product: the FDA withdrew approval of Oxandrin and the referencing generics, having determined they were withdrawn for reasons of safety or effectiveness. Anything circulating now is an unapproved drug.
