Reading a supplement label: tiers, upper limits, and the interactions nobody mentions
Essential is not the same as worth taking, Strong evidence is not the same as it works for you, and the fastest route to a real problem is stacking three products that each contain the same thing.

The supplement aisle is the one place in this app where almost everything is legal, cheap, and available without a conversation. That makes it feel low-stakes. Several of the entries in this catalog carry sharper safety facts than the peptides do.
"Essential" and "worth supplementing" are different claims
Every vitamin and mineral is essential — that is what the word means. It does not follow that taking more helps. Calcium is essential and the supplement evidence for fracture prevention is mixed and population-dependent. Vitamin E is essential, and in the SELECT trial the studied formulation increased prostate-cancer incidence in relatively healthy men (a finding not reproduced in every trial or formulation). Multivitamins fill genuine dietary gaps and have repeatedly failed to change mortality in people who did not have one.
The question a label cannot answer is whether YOU have a gap. Need depends on diet, life stage, medical history and, where appropriate, clinical testing — and it is why iron should be taken for a documented need or a recognised life-stage indication under clinical guidance, not on a guess.
What the evidence tier means
Each entry carries one of four tiers, and the tier describes the KIND and SIZE of the human evidence for the specific use described — never a recommendation.
- Strong — multiple large human trials broadly agree, for that population and that outcome.
- Moderate — real human trials exist, smaller or less consistent.
- Limited — little human data: early, small, or narrow.
- Anecdotal — mostly reports and preclinical work rather than human trials.
A Strong tier on a nutrient often means "correcting a deficiency works", which is a different sentence from "taking it while replete helps". Read the note under the tier; that is where the distinction lives.
Upper limits, and how people quietly exceed them
Fat-soluble vitamins and several minerals accumulate. The usual route to trouble is not one product taken foolishly, it is three products taken sensibly:
- A multivitamin, a dedicated mineral, and a food source can stack past the tolerable upper limit without any single label looking unreasonable. Selenium and preformed vitamin A are examples.
- Desiccated liver and organ capsules carry preformed vitamin A, copper and iron that no label frames as a dose.
- Long-term high-dose zinc depletes copper. That deficiency causes anaemia that does not respond to iron and a myelopathy that may not fully reverse — and the person taking zinc for "immune support" has no reason to suspect it.
The interactions that actually matter
Most supplement interaction talk is theoretical. These are not.
- Biotin can interfere with lab tests. High-dose biotin interferes with certain immunoassays, including some troponin and thyroid tests, and can produce falsely high or low results — the FDA has warned this contributed to a missed heart attack. Whoever orders your bloodwork needs to know you take it.
- Vitamin K opposes warfarin directly. Anyone anticoagulated needs their prescriber involved before starting or stopping it.
- Vitamin B6 causes nerve damage at doses that are widely sold. Sensory peripheral neuropathy from chronic high-dose B6 is well documented and is not always reversible. B6 appears in most B-complexes, energy drinks and multivitamins at once.
- Potassium can stop a heart. Many potassium tablets contain relatively little, while powders and salt substitutes (mostly potassium chloride) can supply far more. Excess potassium causes dangerous hyperkalaemia, especially with kidney disease or interacting medications.
- DIM lowers tamoxifen metabolites, including endoxifen, in a randomised trial. That is an interaction with a cancer therapy.
- Glucosamine and chondroitin have case reports of raised INR on warfarin — a more important question than the shellfish-allergy one people usually ask.
Timing claims are mostly softer than they sound
Where a timing note exists in this catalog it describes what published studies did, not what you should do. Fat-soluble vitamins absorb better with fat — that one is real. Most of the rest ("morning", "empty stomach", "before bed") is convention rather than trial evidence, and the entries say so rather than inventing a schedule.
The honest summary
Fix a measured deficiency and the effect can be dramatic. Take the same product without one and the most likely outcome is an expensive null, with a small tail risk of the accumulation problems above. Neither of those is a reason to avoid supplements; both are reasons to know which situation you are in.
